Application for Dues Relief
This form is used by a Daughter 85 years or older, who is in deteriorating health, to apply for relief of her obligation to pay dues. It must be signed by her clergy and chapter president, or Diocesan or Provincial president if she is a Daughter at Large. It should be returned to the National Office.

Disposition of Cross Card
This file is a two sided file printed on 8 1/2 x 11 paper. Print two copies per member: one stays with the Daughter’s personal papers, and the other is kept with your Chapter’s files. It is recommended that they remain in a file in the church office.

Transition Form
Use this form to report a change of name, address, transfer to new chapter, reinstatement, resignation or member’s death. Please send form to the National Office.

Variable Address Form
Complete this form and submit to the National Office if you have two different homes and are at them the same time each year. (ie: snow birds)
